Compensation
- $35.00 – $43.00 per hour, depending on experience
- PRN / as-needed scheduling
Service Area & Schedule
Cleveland / Northeast Ohio | Flexible scheduling available.
What You'll Do
- Provide skilled nursing care to hospice patients in their homes in accordance with the established plan of care
- Conduct assessments and monitor patient symptoms, comfort levels, and changes in condition
- Administer medications and treatments focused on comfort and quality of life
- Serve as a key point of contact for patients, families, and the interdisciplinary care team
- Provide education and emotional support to patients and families navigating end-of-life care
- Complete timely and accurate documentation in the EMR within required timeframes
- Participate in on-call responsibilities as scheduled

Requirements
Must-Have:
- Active RN license (Ohio) + current BLS certification
- Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area
- Minimum 1 year of clinical nursing experience
Preferred / Nice-to-Have:
- Prior hospice experience
- Experience with end-of-life care and comfort-focused care delivery
- Familiarity with HCHB or similar home health/hospice EMR
- Strong communication and family support skills
