Nursing Supervisor - Medical Respite
PHMC serves as both a direct service provider to individuals, families, and communities across the region and as an intermediary agent managing large-scale contracts, government and philanthropic partnerships, and multidisciplinary initiatives that require operational sophistication, strategic leadership, and deep mission alignment.
The Nursing Supervisor Medical Respite will supervise nursing staff supporting clinical services in post-acute medical care to individuals released from a hospital stay following an illness, surgery, or an injury, and are too ill or frail to recover in a shelter-based setting. Participants are provided with respite-based services, including medication management, discharge planning, appointment transportation and scheduling, case coordination, and connecting clients to primary care. This individual will work closely with the Medical Director, Respite Program Administrator, and other respite staff to support daily respite operations, working relationships with referral sources, on-site patient evaluations, supporting clinical care and staffing in all respects, and secondary support with other administrative matters like IT and food service. All services are provided in collaboration with on-site or community-based case managers and with any other healthcare for the homeless (HCH) staff providing services to the target population. The Nursing Supervisor Medical Respite will report to the Director of Nursing and serve on PHMC Health's nursing leadership team, along with the Nursing Supervisors for Shelter Nursing and Health Centers.
Supervision and management Participate in the management of the team in coordination with PHMC Health leadership, in particular the Medical Director, Director of Nursing, Respite Program Administrator, and other nursing supervisors. Developing strong internal and external partnerships. Supervise and support the nursing staff, including RNs, LPNs, and CNAs. Convenes and facilitates weekly team meetings to review patient progress and for discharge planning. Works in collaboration with social services (case manager/behavioral health). Assist in the development and enforcement of clinical protocols in collaboration with the Director of Nursing and Medical Director. Assist in the development and implementation of all medication storage and management protocols. Manages inventory and purchasing of nursing/medical supplies. Observes HIPAA requirements and provides patient privacy within the limitations of the site. Provide medication management using specified guidelines outlined by the medical respite standard in accordance with the medical respite standards. Support clinical providers and social services staff on regular care, admissions, and discharge goal plans. Review medical respite criteria for admissions with referring entities. Review referrals working in collaboration with the Respite Program Administrator and clinical staff using established criteria to determine appropriateness for the program. Update and maintain daily census. Assist with patients' transportation. Assures the safety of all staff and patients. Coordinates with nursing staff and the Respite Program Administrator for the scheduling of medical appointments. Assists in the coordination of Medication-Assisted Therapy, IV antibiotics, oxygen, wound care, and other more specialized clinical patient care needs. Participates in professional trainings, webinars, etc. Maintain knowledge of programs offered and take responsibility for keeping current with respite program changes and new services offered. Participate in advocacy and coordination committees addressing issues related to homelessness. Assume new responsibilities as new needs are identified.
Infection Control/Emergency Preparedness: Provides coordination and monitoring services for reporting of suspected or confirmed communicable diseases that can be transmitted through routine contact, and provides on-call services for all respite services in partnership with the Respite Program Administrator. Monitor respites for consistent hand washing, care of the environment, use of safety syringes and sharps disposal, and use of personal protective equipment. Manage or participate, as needed, in respite-based communicable disease outbreaks, emergency preparedness/disaster response, and other health initiatives. Provide education to the community and academic health care professionals to provide comprehensive, culturally competent, continuous care to people experiencing homelessness. Provides monthly tracking and record keeping on-site for the environment of care report monthly; and discuss and monitor remediation of any deficiencies with PHMC Health leadership staff.
Clinical: Provides resources for screening and immunization to respite staff, as indicated. Document all services promptly. Reviews, records, and submits monthly CLIA lab reports. Oversees seasonal influenza vaccine process in collaboration with site nurse(s). Maintains site sign requirements to meet Joint Commission requirements. Monitors site equipment maintenance. Attends UDS measures meetings as requested. Supports effective utilization of the Electronic Medical Record for patient care. Conducts auditing of nursing documentation; provides feedback and corrections. Routinely monitor and report outcomes based on HCH data, MARS, on-site logs, and chart review, and develop action plans to address issues identified.
PHMC COMPLIANCE RESPONSIBILITIES: Understands and adheres to PHMC compliance standards as they appear in the PHMC Code of Conduct, Whistleblowers' and Conflict of Interest Policies. Keeps abreast of all pertinent federal, state, and PHMC regulations, laws, and policies as they presently exist and as they change or are modified. Comply with HIPAA and Confidentiality Policies and Procedures as they apply to the job. Comply with Department of Public Health (DPH), The Joint Commission, and other accreditation and regulatory agencies standards. Adhere to all PHMC Policies and Procedures. Knowledge and adherence to Infection Control and Environment of Care Guidelines and Procedures as described in the annual education module.

Education: BSN or MSN preferred Licenses and certifications: Pennsylvania RN
