Physical Therapist - Home Health

Key Physical Therapist Responsibilities Conduct comprehensive assessments to develop individualized therapy plans for patients. Implement therapeutic interventions to enhance patient mobility, strength, and functionality. Educate patients and families on therapy goals and techniques for home-based care. Collaborate closely with a multidisciplinary team to ensure cohesive patient care. Maintain accurate documentation of patient progress and modify therapy plans as needed. Ensure patient safety and adhere to all health regulations and company policies. Work Schedule and Travel Area Schedule (Including any on call/weekends): M-F 8am-5pm; full time; 1 weekend/month; 2 holidays/year; no on call. This position supports patients in Montoursville and surrounding areas. Essential Qualifications Valid PT license in the state of practice with Bachelor's Degree Minimum of 2 years of experience in physical therapy, with preference for home health experience. 1+ years of experience in EMR, Other EMR and Practice Partner EMR Valid DL Strong understanding of home health therapy protocols and patient education. Excellent communication, organizational, and documentation skills. Ability to work independently and adapt to various patient needs.
