Position Summary We are seeking a dedicated Speech-Language Pathologist (SLP) to evaluate, diagnose, and treat residents with speech, language, cognitive-communication, and swallowing disorders. The ideal candidate is passionate about helping older adults achieve their highest level of independence through individualized, evidence-based therapy. Key Responsibilities Resident Evaluation & Treatment Perform comprehensive evaluations of speech, language, cognitive-communication, voice, and swallowing disorders. Develop individualized treatment plans based on physician referrals, clinical findings, and resident goals. Provide skilled speech-language pathology services through individual and group therapy sessions. Evaluate and treat dysphagia using evidence-based assessment and intervention techniques. Monitor resident progress and adjust treatment plans to maximize functional outcomes. Collaboration & Care Coordination Work closely with physicians, nurses, occupational therapists, physical therapists, dietitians, and other interdisciplinary team members to coordinate comprehensive resident care. Participate in care conferences, discharge planning, and quality improvement initiatives. Educate residents, families, and caregivers on communication strategies, swallowing precautions, adaptive techniques, and home exercise programs. Provide training and guidance to nursing staff regarding safe swallowing practices and communication support. Documentation & Compliance Complete timely and accurate documentation, including evaluations, progress notes, care plans, recertifications, and discharge summaries. Maintain compliance with Medicare, Medicaid, state, and federal regulations, as well as facility policies and professional standards. Ensure documentation supports quality care and appropriate reimbursement. Professional Excellence Stay current with advances in speech-language pathology through continuing education and professional development. Participate in departmental meetings, in-services, and facility education programs. Contribute to a positive, collaborative rehabilitation environment focused on exceptional resident outcomes. Qualifications Master's degree in Speech-Language Pathology from an accredited program. Current North Carolina Speech-Language Pathologist license (or eligibility for licensure). Certificate of Clinical Competence (CCC-SLP) from ASHA preferred; Clinical Fellows (CFY) are encouraged to apply if applicable. Experience in skilled nursing, rehabilitation, or long-term care is preferred but not required. Knowledge of dysphagia management, cognitive-communication therapy, and geriatric rehabilitation. Ability to stand, walk, bend, and sit for extended periods throughout the workday. Ability to lift up to 50 pounds when necessary.
