Admission RN- Home Health (4- 8 hour days Friday/Sat/Sun/Mon)

Position Overview We are currently seeking a skilled and compassionate Registered Nurse (RN) to join our home health team. As an Admission RN, you will play a vital role in delivering personalized healthcare services to our patients in the comfort of their own homes. You will work closely with a team of dedicated professionals to provide exceptional medical care, promote wellness, and ensure the highest level of patient satisfaction. This position is a weekend schedule. Responsibilities Work alongside Case Managers to initiate individualized care plans for patients. Administer medication, perform wound care, and provide skilled nursing services according to physician orders. Set the tone for the patient’s home health experience. Review consents and assess for urgent patient needs. Educate patients and their families on disease management, treatment plans, and self-care techniques. Document patient information accurately and in a timely manner using electronic medical records (EMR) systems. Coordinate care with other healthcare professionals, including physicians, therapists, and social workers. Adhere to all relevant legal, ethical, and professional standards, ensuring patient confidentiality and privacy. Qualifications Valid and active California Licensed Registered Nurse (RN) license. Minimum of 1 year of experience in home health, acute care, or a related healthcare setting. Strong clinical skills, including wound care, catheter care, IV blood draw, medication administration, and patient assessment. Excellent communication and interpersonal skills, with the ability to establish rapport with patients and their families. Proficiency in using electronic medical records (EMR) systems. Compassionate and patient-centered approach to care. Reliable transportation and a valid driver's license.
