Physical Therapist

Provides general physical therapy evaluations and treatments that impact patients' functional abilities. Oversees and directs the work of physical therapist assistants and/or designated staff members. Provides general physical therapy evaluations and treatments to a wide range of ages with varying diagnoses and issues. Performs evaluations and identifies therapy diagnoses. Formulates therapy treatment plans with goals and implements physical therapy treatment services for patients. Supervises and modifies therapy plans of care based on assessment of patient response to optimize outcomes. Reviews clinical records and case histories. Attends care conferences, meetings, and remains in contact with referring providers and other healthcare professionals. Collaborates with team members to ensure appropriate medical equipment is arranged and care plans are established prior to discharge. Documents patient encounter findings and treatments, including diagnoses, treatment plans, recommendations, and prognoses. Provides services in a timely manner and according to regulatory requirements. Works with inpatient and outpatient populations in various settings as assigned. Directs the work of support staff, students, or interns. Facilitates a healthcare partnership between providers, patients, and their families. Develops, implements, and conducts patient education and therapy programs based on patient and family needs. Provides patients and families with clear home exercise programs to achieve rehabilitative potential. Ensures that instructions are understood by asking for feedback and return demonstrations from patients or caregivers. Advises patients and family members on methods that facilitate self-care and independence. Creates clear and concise home programs and confirms understanding through feedback and demonstrations. Assists senior staff in administrative matters, clinic operations, and specialty/research areas as appropriate. Utilizes the hospital computer system to obtain patient information and to keep abreast of hospital directives. Participates in the timely and efficient flow of information via email and other digital platforms. Provides summaries of results, statistics, and metrics as required by management. Assists in acquisition and fitting of durable medical equipment/devices based on patient needs. Communicates with supervisors or equipment providers to measure, fit, and deliver equipment, maintaining appropriate stock levels. Maintains supply stocks and clinical equipment at acceptable levels. Remains current in billing procedures for reimbursement of services and dispensable supplies. Maintains cleanliness of clinical spaces. Educates staff on physical therapy-related issues and educates patients and families about functional limitations regarding injuries or diagnoses. Assists in committee and management duties and represents the rehabilitation services department as requested. Provides therapy consultations, presentations, and in-services to medical staff and other healthcare personnel. Adheres to regulatory and practice standards of physical therapy. Follows confidentiality and quality assurance standards; participates in quality management functions. Performs other duties as assigned.
