Position Overview


Seeking a Primary Care Nurse Practitioner (NP) to provide comprehensive, community-based healthcare services to seniors through a Program of All-Inclusive Care for the Elderly (PACE).


Under the direction of the Medical Director, the provider will collaborate with physicians and an interdisciplinary team (IDT) to complete patient assessments, develop individualized care plans, coordinate healthcare services, and educate participants and their families.


This full-time position emphasizes holistic, value-based care, smaller patient caseloads, and long-term patient relationships that help seniors remain in their homes and communities.

• Oversee and Coordinate Comprehensive Patient Care Services
• Provide Patient and Family Education
• Coordinate Discharge Planning From Acute Care and Long-Term Care Settings
• Assist in Maintaining Patient Care in Skilled Nursing Facilities
• Conduct Residential and Skilled Nursing Facility Visits Based on Patient Needs
• Review Clinical Outcomes and Healthcare Utilization Data
• Evaluate Hospitalization Trends and Specialist Utilization
• Identify Opportunities to Improve Clinical Practices and Policies

Requirements

• Graduation From an Accredited School of Nursing - Required

• At Least 2 Years of Clinical Experience in Primary Care or Geriatric Care - Required
