LPN Charge Nurse

The LPN Charge Nurse is responsible for clinical and administrative tasks in a single home setting for one individual requiring 24/7 skilled care. This young adult is engaged in her community and leads a full lifeas long as her medical needs are managed by a skilled nurse. The Charge Nurse is responsible for coordinating care with the LPN team at the home, physicians and other community healthcare providers, and non-clinical Georgia Options staff who assist the individual to live as independently as possible.

This is a Fulltime Monday-Friday Day shift position

ESSENTIAL FUNCTION AND RESPONSIBILITIES

• Carry out nursing care according to the care plan
• Assess for and identify potential health problems, report changes in condition
• Complete and maintain electronic documentation of health status
• Manage, maintain, and administer medications and G/J tube feeding
• Manage medical care with other providers, including staff LPNs
• Manage equipment, medical supplies, and other specialized needs
• Advocate for individual as necessary
• Serve as liaison for family and Georgia Options
• Assist Direct Support Professionals to provide community living support and non-skilled personal care
