Job Summary:
Home Health Physical Therapist (PT) position located in Medford, Oregon. Candidates are required to bring their license on the first day. The role involves providing skilled home health services with a focus on outpatient rehabilitation and patient education. The schedule consists of 8-hour daytime shifts.
Job Details:
- Location: Medford, OR
- Required Qualifications: BLS certification, 1 year of Skilled Home Health experience, Oregon state license, reliable transportation.
- Schedule: 8-hour days, minimum of 40 hours per week.
- Additional Information: Must have experience with electronic charting systems (EPIC/Oasis). Home health experience preferred. The traveler should be prepared for occasional maximum radius of 50+ miles for client visits. The facility cannot accommodate 4x10s or 3x12s shifts. Candidates should have their own transportation, and recent Oasis experience is preferred. References: Two references covering 1 year (6 months for CNAs) within the last 3 years are required. Vaccinations: COVID booster required.
