Clinical Pharmacist - Home Infusion and Patient Care Leader

This role includes assessing therapy appropriateness, documenting patient progress, and coordinating services while adhering to company policies and regulations. The ideal candidate will have a Bachelor of Science in Pharmacy and must be a licensed pharmacist in Texas with at least two years of experience in an acute-care setting or home infusion therapy.
