Key Responsibilities:

• Conduct thorough assessments and develop customized physical therapy treatment plans.

• Apply therapeutic interventions to enhance mobility, strength, posture, gait, and balance.

• Fit and instruct residents on the use of adaptive equipment such as orthotics and prosthetics.

• Select appropriate physical modalities based on residents' diagnoses and physical capabilities.

• Collaborate with a multidisciplinary team to ensure cohesive, resident-centered care.

• Monitor progress, adjust therapy plans as needed, and educate residents and caregivers.

• Contribute to care planning by participating in interdisciplinary meetings.

• Supervise and train support staff, ensuring high standards of therapy delivery.

• Lead in-service education sessions for nursing and medical team members.

• Oversee the ordering and maintenance of therapy supplies and equipment. 

Qualifications:

• Active, unencumbered New Jersey Physical Therapist license .

• Strong communication, organization, and teamwork skills.

• Passion for working with seniors and enhancing quality of life through rehabilitative care.
