Responsibilities
- Evaluate and treat patients using medically prescribed physical therapy programs to restore function and minimize disability
- Supervise Physical Therapy Assistants when appropriate; participate in initial admissions/evaluation visits
- Document observations, clinical findings, problems, skilled interventions/treatments, goals, and discharge plans
- Complete and timely documentation, including OASIS scores, and notify the Clinical Team Manager within established timelines
- Maintain thorough patient records and discharge planning, adjusting plans as progress is made
- Collaborate with an interdisciplinary team to deliver patient-centered care aligned with home health and hospice goals

Requirements
- One year of Home Health experience
- Ability to work in a home health/hospice setting and travel within the defined territory
- Strong documentation skills, including experience with skilled interventions and discharge planning
- Ability to perform initial admissions/evaluation visits and coordinate OASIS scoring within established timelines
- Effective communication and teamwork to collaborate with the interdisciplinary care team
