Join our elite team of healthcare professionals, providing the Right Care, at the Right Time, in the Right Place.**Job Description:**## Make a Difference Beyond the Clinical VisitElara Caring is hiring a **part-time Medical Social Worker** to support home health patients and families throughout Portland and surrounding southern Maine communities.This is meaningful work. You will help patients navigate emotional, social, financial, and environmental challenges that may affect their ability to remain safely at home. You will work independently in the community while receiving support from an experienced interdisciplinary team.## ## Why Join Elara Caring?* One-on-one care in the patient’s home* Meaningful relationships with patients and families* Supportive interdisciplinary clinical team* Independence in the field* Continuing education opportunities* Tuition reimbursement for eligible employees* Opportunities for professional growth* 401(k) with company match* Medical, dental, vision, paid time off, and additional benefits based on eligibility## ## What You’ll Do* Assess patients’ psychosocial, financial, and environmental needs* Complete initial social work evaluations and follow-up visits* Provide counseling, education, and support to patients and caregivers* Connect families with community programs and financial resources* Participate in developing and updating individualized Plans of Care* Collaborate with physicians, nurses, therapists, and other care team members* Communicate changes or concerns affecting patient safety and well-being* Support discharge planning and continuity of care* Complete accurate and timely clinical documentation## ## What You’ll Bring* Master’s or Doctoral degree in Social Work from a program accredited by the Council on Social Work Education* Current Maine Social Worker license in good standing* At least one year of social work experience in a healthcare setting* Strong communication, counseling, documentation, and advocacy skills* Ability to work independently in patients’ homes* Willingness to travel throughout the assigned service area with 50% travel required* Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment* Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations## ## Ready to Make an Impact?If you want to use your social work experience to help patients overcome barriers, access essential resources, and remain safely at home, we would love to meet you.**Apply today and help us care where our patients call home.**
