Director of Transitional Care & Nursing Leadership
SUNY Upstate Medical University
SUNY Upstate Medical University is seeking a Director of Transitional Care to develop and coordinate Transitional Care Initiatives. This role includes managing operations, engaging with the community, and ensuring staff satisfaction and quality management. The ideal candidate will have a Master's degree in Nursing or Social Work, be a licensed RN or have LMSW, and possess at least 7 years of nursing leadership experience.
