Job TitleOur Home Care RN'sJob DescriptionCase manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.Measure vital signs, Administer IVs, change dressings, clean wounds as neededProvides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.Provide patient and family education for disease management and health promotion as directed by physician's plan of treatmentProvide care to the most upmost standardMinimum QualificationsNew York State RN licenseMinimum of one year of recent hands on experience as a Registered Nurse.Basic Life Support CertificationValid New York State Driver's License and reliable transportationPosted Salary RangeUSD $35.00 - USD $135.00 /Yr.The pay for this position ranges from $35 – 135 Hourly. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience.
