Registered Nurse, Home Health

Provide home health nursing care to patients in their preferred home setting Assess assigned caseloads of home health care patients and families Develop, implement, coordinate, and evaluate patient plans of treatment Assess physical, psychological, social, spiritual, and environmental patient needs Conduct initial evaluations and re-evaluate nursing needs during visits Make follow-up visits according to patient, family, and facility needs Communicate significant findings, problems, health-condition changes, and unsafe conditions to the Clinical Supervisor, physician, facility, and care team Revise plans of care in response to patient-care issues and notify relevant stakeholders Maintain patient and staff privacy and confidentiality under HIPAA Report to the Clinical Manager or Clinical Supervisor Requirements Current, unrestricted state RN license 1+ year of experience in a clinical care setting as a nurse Ability to frequently lift, push, pull, and support up to 50 pounds Ability and willingness to travel within the branch/office coverage area Dependable vehicle Valid driver’s license and current auto insurance in accordance with state laws Ability and willingness to travel 50% Associate’s Degree or Bachelor’s Degree in Nursing preferred Previous Home Health or Hospice experience preferred Previous experience with HomeCare HomeBase preferred Must maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule Core Competencies Demonstrates expertise in providing home health nursing care, including patient assessment, treatment planning, and coordination of care while ensuring compliance with HIPAA regulations.
