Overview
Physical Therapist needed for a travel assignment in a Skilled Nursing Facility setting in Waterville, Maine. Start date: December 1, 2026. Duration: 13 weeks. Weekly pay: $2,366–$2,500 per week, with Guaranteed Hours: 35 per week. Shift: 5x8 Days (8:00 a.m. to 4:30 p.m.). This role provides direct PT services to residents in a SNF, focusing on rehabilitation and functional mobility within a geriatric care environment.

Responsibilities
- Conduct initial evaluations and ongoing reassessments to guide individualized treatment plans
- Deliver PT interventions including therapeutic exercises, gait and balance training, ROM advancement, and functional mobility activities
- Monitor progress, adjust treatment plans, and document outcomes clearly in resident medical records
- Educate residents and families on home exercise programs, safety strategies, and fall prevention
- Coordinate care with nursing, therapy colleagues, and the interdisciplinary team to optimize rehabilitation goals
- Ensure patient safety, apply infection control practices, and maintain compliant documentation and regulatory standards

Requirements
- Active Physical Therapist license in the United States (eligible for licensure in Maine as applicable)
- Ability to work the stated shift schedule in a Skilled Nursing Facility environment
- Strong assessment, documentation, and communication skills
- Commitment to a 13-week travel assignment starting 12/01/2026
