What You'll Do Provide skilled hospice nursing care including medication administration, symptom management, wound care, and patient monitoring. Observe changes in patient condition and communicate with the RN Case Manager and interdisciplinary team. Educate patients and caregivers while providing compassionate, relationship-centered care. Deliver services in private homes, assisted living communities, memory care, and skilled nursing facilities. Maintain accurate EMR documentation. Participate in weekend, holiday, and on-call rotations as scheduled. What You'll Bring Active Minnesota Licensed Practical Nurse (LPN) license. Previous experience in hospice, home health, long-term care, skilled nursing, oncology, or palliative care . Strong communication and clinical assessment skills. Ability to work independently while collaborating with an interdisciplinary team. A compassionate, dependable, and patient-focused approach to care.
