Seven Healthcare are seeking an experienced RN specializing in Case Management for a travel assignment in Peoria, IL. This role requires collaboration within an inpatient physical rehab or LTAC facility. The chosen candidate will be responsible for ensuring patient progression through the continuum of care, from admission through discharge, and will effectively manage resources in an acute hospital setting.

- Development of individualized care plans from admission through discharge.
- Management of patient resources efficiently and cost-effectively.
- Conduct interdisciplinary rounding and family meetings.
- Engage in utilization reviews and handle insurance authorizations.
- Coordinate discharge planning and make appropriate referrals for DME, HH, or outpatient services.
- Registered Nurse License or Social Worker License (LSW/LCSW) in Illinois
- Basic Life Support (BLS) certification
- A minimum of 2 years of experience in a healthcare setting, preferably in acute care
- Experience in discharge planning and care coordination
- Proficiency in EPIC charting system
