Registered Nurse (RN) Case Manager

We are seeking a dedicated and experienced Registered Nurse Case Manager to join our team. The ideal candidate will possess a strong background in care management and be certified as a Case Manager. This role is crucial for overseeing and facilitating patients' care plans, ensuring high-quality patient outcomes.

Key Responsibilities

• Manage a diverse caseload of patients, coordinating their care from initial assessment through discharge.
• Develop and implement individualized care plans in collaboration with healthcare teams.
• Utilize best practices in case management to enhance patient satisfaction and outcomes.
• Act as a patient advocate, providing education and support to patients and their families.
• Collaborate with interdisciplinary teams to address patient needs and optimize resources.
• Monitor and evaluate patient progress, adjusting care plans as necessary.
• Report and document all patient interactions accurately in compliance with policies.

Qualifications

• Education: Bachelor's Degree in Nursing (BSN) preferred.
• Experience: Previous experience in a care management role is preferred, especially with a focus on inpatient case management.
• Licenses/Certifications:
  • Active Registered Nurse (RN) licensure in the state of practice is required.
  • Case Management Certification (preferred).
  • Basic Life Support (BLS) certification from an approved vendor is required.
• Familiarity with healthcare policies and practices related to case management.

Work Schedule

This position requires availability for day shifts from 8:00 AM to 4:30 PM, Tuesday through Saturday, amounting to a total of 40 hours per week. There is no on-call duty associated with this role.

Preferred Skills

• Experience with strike force operations, especially in a Kaiser Permanente or HCA California setting.
• Ability to manage a high volume of cases effectively and efficiently.
• Strong communication and interpersonal skills for effective patient interaction.
