Home Health RN/LPN – Job Description

Job Summary
We are seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Home Health Team.
Full-time, Part-time, and PRN positions .  In this role, you will provide high‑quality, patient‑centered care to an adult or a pediatric client in the comfort of their home. The ideal candidate brings strong clinical skills, physiology knowledge, and experience in hospital, skilled nursing, home care, or community care settings. 

This is an opportunity to provide individualize care and make a meaningful impact on the client and their family. While supporting the client in maintaining safety, independence, and dignity at home.

Responsibilities

•              Work12‑hour shifts, including overnights and weekends as needed.
•             Provide direct skilled nursing care to adult and pediatric clients in their homes .
•              Administer medications, monitor vital signs, and assist with daily living activities .
•             Develop and implement individualized care plans to ensure safe, effective, and patient‑centered care .
•             Collaborate with physicians, nurses, and interdisciplinary team members to support comprehensive care coordination.
•             Document all care accurately using electronic medical records (EMRs).
•             Utilize WellSky EMR (experience preferred; training provided).
•             Maintain timely and accurate clinical documentation, including progress notes and discharge summaries. 
•            Professionalism, strong communication skills, and respectful interpersonal skills.

Requirements

•             Current Florida RN or LPN license .
•            Experience in a hospital, skilled nursing, or home/community care setting.
•             Strong physiology knowledge and experience with airway management.
•             Ability to travel to patients’ homes and provide care.
•             Ability to work effectively in a fast‑paced environment while delivering high‑quality patient care.
