Job Summary


Under general supervision of the Department, assesses patient and family Psycho-Social issues relevant to medical treatment and potential Discharge Planning issues. Provides crisis intervention, emotional support, resource information, discharge planning assistance and legal reporting. Works within the team framework to provide alternative plans and resources to facilitate a smooth transition for the patient and family from prior baseline, to acute admission, transitioning back to prior baseline. Assesses all admitted patients with psychiatric hold criteria when requested and addresses concerns or request for assistance by medical providers. Assists providers in evaluation and disposition of patients on psychiatric hold (5150).


Duties

• Performs psychosocial assessments and assists the Case Manager in appropriate discharge planning for high risk patients.
• Participates in patient care planning responsive to socioeconomic, age specific, gender, racial, sexual orientation and cultural preferences that may affect the needs and differential use of services.
• Refers patients to appropriate community resources.
• Provides crisis intervention and emotional support to patients and families on medical issues.
• Provides consultations to physicians and hospital staff regarding psychosocial issues and barriers to discharge.
• Develops and maintains working relationships with community agencies.
• Provides in-service education in social work field when requested by staff.
• Documents pertinent contacts regarding discharge plans to the Social Services notes of the patient's medical record.
• Performs basic administrative tasks related to the job which may include updating community resource lists, statistics and other routines as required.
• Performs legal reports for suspected abuse, specific to mandated reporting for children and elders, appropriate domestic violence referrals.
• Upholds all regulatory guidance regarding confidentiality and consent.
• Performs other duties as assigned.
