About This Role The Behavioral Health Associate provides community-based professional nursing care to patients in various settings, including group and private homes, as well as those without housing across New York City. This role is ideal for individuals who are skilled in nursing methods and techniques, focusing on the health and well-being of patients. Highlights Pay: $116,159/year Location: New York, NY Department/Unit: AT HOME/HC - ADULT MANHATTAN Schedule: Full-Time, Day shift (9:00 A.M - 5:30 P.M.) What You'll Do Provide services requiring substantial and specialized nursing skill per the treatment plan signed by the physician. Identify and distinguish between physical and psycho-social signs and symptoms. Develop, implement, and continuously evaluate the nursing regimen for assigned patients/clients. Complete comprehensive patient assessments and maintain individualized client care progress records. Record and maintain nursing care plans and progress notes to ensure continuity of care. Interpret and report responses of patients/clients to appropriate members of the health team. Coordinate the total plan of care with other health team members. Maintain continuity of client care by liaising with other health professionals. Determine and assign components of the nursing regimen to allied nursing personnel. Provide teaching, guidance, and direction to allied nursing personnel in patient care.
