Job Summary:

Responsibilities:

• 
  Assess patients' nutritional status and medical history to develop individualized nutrition plans.
  
  
• 
  Educate patients and healthcare staff on dietary requirements and nutritional best practices.
  
  
• 
  Monitor and evaluate patient progress, adjusting plans as necessary to meet evolving needs.
  
  
• 
  Maintain accurate documentation of assessments, interventions, and outcomes.
  
  
• 
  Collaborate with multidisciplinary teams to integrate nutritional strategies into overall care plans.
  
  

Qualifications:

• 
  Hold a current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential.
  
  
• 
  Possess at least 2 years of experience in clinical nutrition or related settings.
  
  
• 
  Demonstrate strong communication skills and ability to educate diverse patient populations.
  
  
• 
  Show proficiency in assessing nutritional needs and creating tailored plans.
  
  
• 
  Comply with all relevant state and federal licensing requirements.
