Why Join the Elara Caring mission? * Work in a collaborative environment. * Be rewarded with a unique opportunity to make a difference. * Competitive compensation package * Tuition reimbursement for full-time staff and continuing education opportunities for all employees at no cost * Opportunities for advancement * 401(K) with employer match * Pet insurance As a Licensed Practical Nurse LPN, you'll contribute to our success in the following ways: * Deliver in-home nursing care aligned with patient care plans * Review and follow Plan of Care prior to each visit * Communicate patient updates, concerns, and changes in condition * Assist with medication management and documentation * Educate patients and families on self-care techniques * Identify safety concerns and help prevent hospitalizations * Collaborate with RN Case Managers, physicians, and care team What is required? * Current LPN/LVN license, in the state of Missouri * Graduate of an accredited nursing program * At least 1 year of experience in acute care ( home care, home health or hospice preferred) * Strong communication and interpersonal skills * Ability to manage time effectively and stay organized in the field * Knowledge of healthcare regulations and patient care standards * Compassionate, patient-first mindset * Reliable personal transportation (travel required ~50%) * Ability to meet health screening and compliance requirements You will report to the Clinical Manager or Branch Director for Personal Care Services.
