- Conduct thorough evaluations of patients in both inpatient and outpatient settings, formulating personalized treatment plans
- Implement a range of physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality
- Partner with a multidisciplinary team of nurses, physicians, case managers, and rehabilitation specialists to achieve the best possible outcomes
- Engage with patients and their families to establish clear therapeutic goals, assist with discharge strategies, and share essential home exercise routines
- Maintain comprehensive documentation of evaluations, treatments, and patient progress following facility and regulatory protocols
- Actively participate in care team discussions and interdisciplinary meetings to promote coordinated patient care
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
- State licensure as a Physical Therapist
- CPR certification is preferred
- Additional certifications (neurological, geriatric) are advantageous but not mandatory
- Strong preference for prior experience
- Demonstrated capability to manage patients with intricate medical and mobility challenges
- Excellent interpersonal and collaborative skills
- Familiarity with digital documentation and clinical software systems
