About the Role:
Panacea Home Healthcare LLC is looking for a ONE-ON-ONE dedicated and compassionate RN or LPN to join our growing team in White Oak, OH. This is a rewarding opportunity to make a real difference in patients' lives by delivering high-quality, personalized care right in the comfort of their homes. If you're a skilled nursing professional who thrives in an independent yet supportive environment, we want to hear from you! Hours needed are Monday through Saturday 6am to 10am. - Provide direct patient care in home settings, including assessments, treatments, and medication administration
- Develop, implement, and monitor individualized patient care plans
- Monitor and document patient health status, progress, and changes in condition
- Communicate effectively with physicians, care teams, and family members regarding patient needs
- Educate patients and caregivers on disease management, medications, and home safety
- Maintain accurate and timely clinical documentation in compliance with agency and regulatory standards
- Active RN or LPN license in the state of Ohio (in good standing)
- Prior home health, long-term care, or clinical nursing experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Reliable transportation and valid driver's license for home visits
- Ability to work independently and manage the patient
- CPR/BLS certification required or ability to obtain upon hire
