Physical Therapist

Role: Licensed Physical Therapist specializing in geriatric care, providing one-on-one therapy in private homes and senior living communities. Responsibilities: Conduct evaluations, develop personalized treatment plans, deliver evidence-based therapy to improve mobility and independence, document in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate. Qualifications: Valid state license or eligibility, degree from an accredited program, proficiency in EMR and basic computer skills, and current CPR certification. Physical Requirements: Ability to perform moderate physical tasks, assist with patient transfers, repositioning, and mobility activities, and drive between locations.
