LPN – Long-Term Care

Schedule: Eve block 2:00 PM - 10:30 PM, 72 hours (9 shifts in 2 weeks)

Requirements:

• Valid LPN License
• Minimum 1 year of experience in long-term care

Roles & Responsibilities:

• Provide direct patient care under the supervision of a Registered Nurse.
• Monitor and record patient vital signs and health status.
• Assist patients with activities of daily living, such as bathing and feeding.
• Administer medications as prescribed.
• Communicate effectively with patients and their families regarding care plans and progress.
• Collaborate with interdisciplinary teams to provide holistic care.

• Must comply with facility policies and procedures.
• Background checks and drug screens may be required.
