Home Care Transition Nurse
The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals.
5 x 8 hour shifts (9:30AM to 5:30PM) - Rotating weekends and holidays

This position is on site in our Farmingdale office located at 110 Bi-County Boulevard
Job Details
Our Home Care Transition Nurses:
- Establish and maintain professional relationships with key physician groups and referral sources.
- Act as liaison to referral sources on behalf of Catholic Home Care.
- Identify potential market growth opportunities and program expansion.
- Serve as a customer service representative and act in a professional and supportive manner at all times.
Requirements
- Registered Nurse with current licensure in NYS
- 2 or more years of home care experience preferred
- Customer service and marketing skills required
Posted Salary Range
USD $90,000.00 - USD $105,000.00 /Yr.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
