SUMMARY: This is a community-based position. The Case Manager/CPST Provider delivers Community Psychiatric Supportive Treatment (CPST) services in accordance with Ohio Administrative Code (OAC) rule 5122-29-17 and the certification standards of the Ohio Department of Mental Health and Addiction Services (OhioMHAS) under Ohio Revised Code (ORC) Chapter 5119. CPST services provide an array of coordinated, individualized supports and interventions that address the mental health needs of the client and are designed to help the individual achieve and maintain stability and independence in the community. Services are delivered primarily in community settings — including the client's home, school, workplace, and other natural environments — as well as in office settings, and are directed toward adults, children, adolescents, and families. Services will vary with respect to hours, type, and intensity, depending on the changing needs of each individual as identified in the Individual Service Plan (ISP)/treatment plan.
SUPERVISOR: Clinical Supervisor or Designee
DUTIES AND RESPONSIBILITIES:
• Provide specific, measurable, and individualized CPST services, consistent with OAC 5122-29-17, to adults with severe and persistent mental illness, children and adolescents with serious emotional disturbance, their families, and other significant individuals.
• Provide services in the community, meeting clients where they are — in their homes, schools, workplaces, and other community locations — with the majority of service delivery occurring outside the office setting, and provide services focused on the individual's ability to succeed in the community; to identify and access needed services; and to show improvement in school, work, family, and integration and contributions within the community.
• Provide ongoing assessment of the client's needs and assistance in achieving personal independence in managing basic needs, as identified by the individual and/or parent or guardian.
• Facilitate further development of daily living skills.
• Provide coordination of the Individual Service Plan (ISP)/treatment plan, including coordinating with services and supports identified in the plan, assisting the client in accessing needed medical, social, educational, vocational, housing, and other services, and communicating with other service providers, family members, and natural supports, consistent with client consent and applicable confidentiality laws.
• Provide symptom monitoring and assistance and coordination in crisis management and stabilization as needed, including participating in the development and implementation of crisis/safety plans and linking clients to crisis services.
• Provide mental health interventions that address symptoms, behaviors, thought processes, etc., that
assist an individual in eliminating barriers to seeking or maintaining education and employment.
• Provide education and training specific to the individual's assessed needs, abilities, and readiness to
learn, and conduct activities that increase the individual's capacity to positively impact his/her own
environment.
• Provide advocacy and outreach on behalf of clients, including activities intended to engage or reengage
individuals in treatment and community supports.
• Complete all clinical documentation — including progress notes, ISPs/treatment plans, and reviews —
accurately, completely, and within required timeframes, in accordance with OAC Chapter 5122,
Medicaid requirements under OAC Chapter 5160-27, and Agency policy.
• At a minimum, achieve and maintain the Agency's established productivity standards.
• Maintain current knowledge of all applicable AFS policies and procedures and function according to
these standards.
• Be knowledgeable of and adhere to all pertinent federal, state, and local laws and regulations, including
the Ohio Revised Code and Ohio Administrative Code, the certification standards of OhioMHAS, client
rights and confidentiality requirements (including OAC 5122-26-18, HIPAA, and 42 C.F.R. Part 2 where
applicable), and appropriate professional ethical standards.
• Comply with Council on Accreditation (COA) standards and all educational, certification, and registry
requirements applicable to the position.
• Participate in regular supervision, team meetings, and required in-service training.
• Other duties as assigned.
The Duties and Responsibilities above represent the most significant duties of this position, but do not

position.
Requirements:MINIMUM QUALIFICATIONS:
• Bachelor’s Degree - counseling or social work, psychology, or another mental health related field.
• Ability to establish and maintain effective relationships with clients, family members, and significant
others.
• Ability and willingness to work in community settings, including client homes, schools, and other
locations where services are needed; ability to travel throughout the service area on a daily basis.
• Valid driver's license, reliable transportation, and adequate levels of insurance and insurability under
Agency policy.
• Must be able to meet Agency requirements for criminal background check (BCI/FBI) in accordance with
ORC 5119.34 and applicable Agency policy.
• Must meet the qualifications to provide CPST services under OAC 5122-29-17 and applicable
OhioMHAS certification standards.
• Prior community support, case management, or CPST experience with individuals with severe and
persistent mental illness or serious emotional disturbance preferred.
• Knowledge of community resources and the public behavioral health system preferred.
• Licensure through the Ohio Counselor, Social Worker, Marriage & Therapist Board (e.g. LSW, LPC)
Preferred.
• Candidates qualifying as a qualified behavioral health specialist under applicable OhioMHAS and Ohio
Medicaid rules, working under required supervision, may also be considered in accordance with those
rules.
LICENSES OR CERTIFICATIONS REQUIRED:
• Unrestricted State of Ohio driver’s license
COMPLIANCE:
This position requires compliance with AFS’ written standards, including its Compliance Program and
Standards of Conduct and policies and procedures. Such compliance will be an element considered
as part of the regular performance evaluation.
Failure to comply with AFS’ Written Standards, which may include the failure to report any conduct or
event that potentially violates legal or compliance requirements or AFS’ Written Standards or, for
managers and supervisors, fails to detect non-compliant conduct where reasonable efforts would
have resulted in detection, will be met by the enforcement of disciplinary action, up to and including
possible termination, in accordance with AFS’ Compliance Program Policy and Procedure:
Addressing Instances of Non-Compliance through Appropriate Disciplinary Actions.

While performing the duties of this job, the employee is frequently required to use close vision (clear
vision at 20 inches or less). The employee will occasionally be working near moving mechanical

running the copier, printer, other standard equipment and answering the telephone. The noise level in
the work environment is low to moderate. Stress level can be high at times.
