Nurse Practitioner - Family Medicine (Full-Time)

The Nurse Practitioner provides continuing and holistic healthcare service to individuals and families. Responsibilities Delivers a variety of healthcare services, including preventative, chronic, and acute services. Performs diagnosis and treatment of illnesses, carries out health-risk assessments, routine check-ups, screening tests and immunizations, and counsels patients on healthy lifestyle choices to prevent disease. Ensures participation in continuing education and training to stay current with industry developments. Provides quality care following evidence-based guidelines and practice procedures. Participates in quality assurance audits. Qualifications Licensed Nurse Practitioner in the State of Illinois with a valid Illinois Controlled Substance License (within 6 months of hire). Eligible for DEA registration and possession of a current DEA registration (within six months of hire). Current ACLS and BLS certifications. Board-certified in Family Medicine, Adult-Gerontology, or related field. Prior experience in primary care and strong clinical assessment, communication, and documentation skills. Commitment to providing high-quality, compassionate care. Bilingual in Spanish strongly preferred. #J-18808-Ljbffr Humboldt Park Health
