Our center is structured to provide high-quality home care services, emphasizing thorough assessments and individualized therapy. You will work alongside a dedicated interdisciplinary team that consists of nursing staff, occupational therapists, and social workers, all focused on holistic patient care.Key ResponsibilitiesPerform extensive evaluations to assess patients' needs, understanding their physical limitations and personal goals in order to create tailored treatment plans.Develop and implement a variety of therapeutic modalities, including manual therapy techniques, therapeutic exercises, and functional training to improve patients' physical function and mobility.Monitor and document patient progress regularly, adjusting treatment plans as necessary to ensure optimal rehabilitation outcomes.Educate patients and their families on therapeutic interventions, exercise regimens, and injury prevention techniques to support recovery and promote long-term health.Maintain accurate records of therapy sessions, patient interactions, and progress to ensure compliance with local regulations and best practices.Participate in regular multidisciplinary team meetings to discuss patient care plans and contribute insights towards improving healthcare services.Patient Population / Client FocusThe primary focus of this role will be on patients recovering from various conditions such as post-surgical rehabilitation, orthopedic injuries, neurological conditions, and age-related impairments. Our patient population ranges from young adults to seniors, each requiring tailored approaches that address their unique rehabilitation needs and recovery goals. You will play a vital role in helping these individuals regain their independence while enhancing their quality of life.QualificationsA current and valid New York State Physical Therapist License is mandatory for this position.A Master's degree in Physical Therapy (MSPT) or a Doctorate in Physical Therapy (DPT) is required.A minimum of two years of experience in a physical therapy setting, preferably in geriatrics or skilled nursing.Preferred ExperienceExperience working within a skilled nursing facility and familiarity with home care rehabilitation services.Proficiency in using physical therapy assessment tools and treatment modalities.Excellent interpersonal and communication skills to effectively engage with diverse patient populations and their families.Knowledge of electronic health record systems for efficient documentation and patient management.Compensation and BenefitsThis position offers a competitive salary ranging from $100,000 to $120,000 per year, commensurate with experience.
