Hospice On Call Visiting Nurse

Hartford Healthcare is seeking a Hospice On Call Visiting Nurse to provide compassionate, expert care to patients in their homes on a 7 days on / 7 days off schedule. In this role, you’ll respond to after-hours hospice needs, perform comprehensive assessments, manage symptoms, administer medications, and update care plans in collaboration with the interdisciplinary team. You will educate patients and families, ensure continuity of care, document accurately in the EMR, and coordinate with physicians and caregivers.
