Moffitt has been recognized as a Best and Brightest Company to Work For in the Nation and is continually named one of the Tampa Bay Times’ Top Workplaces.**Summary**As an important member of the multi-disciplinary care team, the Case Manager acts as a patient advocate by coordinating the care and service of patient populations across the continuum of care; promote effective utilization and monitoring of health care resources by collaborating with the health care team; maintain accurate knowledge of federal, state and payer standards; and assume a leadership role with the interdisciplinary team to achieve optimal clinical and resource outcomes. Core processes include risk assessment and management of clinical populations, discharge planning and utilization management.
