As a Wellness Nurse LPN, you play a central role in monitoring resident health, supporting medication oversight, and maintaining clinical quality across the community. What You’ll Do: • Conduct move-in, 3-day, semi-annual, and change-of-condition assessments • Perform monthly wellness visits and monitor health trends • Follow up on falls, incidents, and hospital returns • Transcribe physician orders and ensure accurate MAR/TAR documentation • Complete medication audits and oversee medication compliance • Partner with families, physicians, and the Resident Care Director • Support infection control and regulatory standards • Educate and guide care staff on documentation and medication practices You help prevent decline through proactive assessment, early intervention, and strong clinical oversight. What We’re Looking For: • Active LPN license in good standing • Graduate of approved nursing program • 1+ year experience in senior care or clinical setting • Strong assessment, documentation, and communication skills Licensed Practical Nurse (LPN/LVN) – Support Resident Health and Wellbeing Licensed Practical Nurse LPN LVN | Nursing | Assisted Living | Senior Living
Pay Range: 29.15 - 38.90
