Hiring Registered Nurses - Quality Improvement #JN26

Overview:

Avid Healthcare Services is hiring Registered Nurses (RN) for assignments with Sacramento County. This Quality Improvement assignment supports the SCHC's compliance with the regulatory requirements related to clinical performance.

Position:
  • Registered Nurse (RN)
Requirements:
  • Current California RN License
  • Current CPR/BLS Certification
Experience:
  • Quality Improvement
Clinical Skills:
  • Knowledgeable about the requirements for Primary Care and the Patient Centered Medical Home Model (PCMH) within a federally qualified health center setting.
  • Knowledge of the Quality Improvement Process and effective tools.
  • Demonstrate clear and effective communication - oral and written.
  • Establish and maintain familiarity with Federally Qualified Health Center (FQHC), Uniform Data System (UDS), and Healthcare Effectiveness Data and Information Set (HEDIS) standards and mandates.
Duties include but not limited to:

Quality Improvement (QI)
  • Participate in care management activities to support patients at high risk as assigned in accordance with current policies and procedures.
  • Sort and organize information in Health Plan Gaps in Care data reports to help establish priorities for improvement and whether process changes resulted in improvement.
  • Discuss and participate in action plans to decrease any gaps in care.
  • Contact patients for appointment reminders as needed or assigned.
  • Review patient charts to monitor clinical performance measures.
  • Generate reports indicating individual Provider panels at regular intervals.
  • Create and present quality data reports to the Quality Improvement Committee and other meetings as assigned.
  • Attend and actively participate in Health Center meetings as required.

Shift:

Monday-Friday
