Home Health Licensed Practical Nurse

Licensed Practical Nurse – Whatcom County Signature Healthcare at Home is seeking a Licensed Practical Nurse to serve the Whatcom County area. The role involves traveling in the field most of the time, with occasional in-person meetings at the company's Bellingham office. Ideal candidates have home health experience or a strong desire to grow within this setting. Responsibilities Provide direct patient care and assist in developing comprehensive care plans. Deliver accurate and timely documentation of patient services. Manage pain/symptom control, evaluate patient responses to treatment/medication, and prepare and use supplies or equipment efficiently. Complete mandatory education requirements, compliance training, and meet productivity expectations within hourly status. Travel as needed outside of the assigned area based on census or current staffing. Drive daily to patients’ homes, complete documentation via smartphone, and participate in case conferences or case management with nursing staff. Qualifications Graduate of an accredited practical or vocational nursing program. One to two years of nursing experience preferred; community health or medical/surgical experience a plus. Currently licensed and maintains licensure in Washington. Valid CPR certification and a valid driver’s license with personal automobile insurance for daily travel. Schedule, Pay and Benefits Full‑time Monday through Friday, day shift. Auto‑schedule patients to set your own day. Hourly $35–$40 with overtime eligibility. Hourly mileage reimbursement. Medical/Dental/Vision coverage starts the first of the month after hire. Paid company holidays, 2+ weeks paid time off, and 401k match after 90 days.
