Behavioral Health Clinician
Alpha Home Health and Hospice
Role: Conduct psychosocial assessments and develop individualized treatment plans. Deliver evidence-based individual, group, and family therapy to diverse populations. Collaborate with psychiatrists, nurses, and primary care providers to coordinate integrated care. Support safe discharge planning and ensure continuity of care.
