Remote Care Manager - Neurology & Chronic Care
GLIA HEALTH MANAGEMENT LLC
The Neuron Clinic is seeking a Care Management professional to deliver CCM and PCM care programs under licensed physician supervision. You will manage a patient panel, engage monthly with patients per the clinician's plan of care, and coordinate services for complex neurological cases including dementia, movement disorders, and infusion therapy.
This full-time role is remotely eligible within California, with preference for CA LVN licensure and prior outpatient care management experience in
