Job Summary A licensed Speech Therapist professional who provides coordinated care to patients, evaluates patients, and restores or maintains patient well-being according to the plan of care. Evaluate and provide a treatment plan for a patient with physical, psycho-social, and/or cognitive impairments. Provide intervention to the deficit and elicit responses. Collaborate with interdisciplinary team members. Job Qualifications Education: Bachelor of Science in Speech Therapy. Licensure: Current Texas State License as a Speech Therapist. Experience 1 year of minimum experience as a Speech Therapist in Home Healthcare, Acute care, or Rehabilitation setting. Skills Therapy skills as defined by generally accepted standards of practice. Good communication and interpersonal skills. Proof of current CPR and Hepatitis profile. Knowledge of durable medical equipment. Spanish is a plus. Essential Functions Perform initial and ongoing clinical assessments to identify patient needs, determine the level of function at appropriate time points, and determine appropriateness of the patient for Home health. Develop and revise a plan of care in consultation with the physician and other care team members. Provide therapeutic treatments, evaluate the need for and use of adaptive equipment and environmental modifications. Evaluate outcomes of care with appropriate discharge planning. Apply concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease. Accept clinical assessments that are consistent with education and competency to care for patients. Supervise as needed. Complete appropriate documentation in a timely manner. Meet mandatory continuing education requirements of the agency/license board. Use effective interpersonal relations and communication skills to advise and consult with the family and other agency personnel as appropriate.
