Job Title: Floating RN Case Manager - Home Health Company: Home Health Care, Inc. Compensation: $95,000 - $130,000 per year + Incentive Visits! Position Summary: In this full-time RN Case Manager role, you’ll assess clients’ health needs, provide skilled nursing care, and guide caregiver training across the Twin Cities Metro Area. You’ll experience the satisfaction of delivering quality care directly to clients in their homes, with the support of a collaborative and dedicated team. You will also move around to help with caseload fluctuations across designated territories to ensure a timely initiation of care and progress clients through their care plan.
