Job Summary

We are seeking a dedicated and compassionate Hospice Registered Nurse for a 13-week contract assignment based in Sunnyvale, California. This field-based role involves providing comprehensive hospice care to patients and their families within a defined territory including Millbrae, Burlingame, San Mateo, Foster City, Belmont, and Redwood City. The successful candidate will play a crucial role in delivering holistic support during a critical time.

The Hospice RN is responsible for admitting new patients, conducting thorough physical, psychosocial, environmental, safety, and developmental assessments. This role involves developing individualized care plans, meticulous documentation of observations and interventions, and coordinating care seamlessly with a multidisciplinary team. Active participation in quality assessment and performance improvement initiatives, along with conducting necessary follow-up visits, are integral to this position.

Key Responsibilities

• Admit patients to hospice service, ensuring a smooth transition into care.
• Conduct comprehensive assessments of the patient’s and family’s physical, psychosocial, environmental, safety, and developmental needs.
• Develop and implement individualized plans of care based on assessment findings.
• Document observations and interventions accurately and thoroughly, including those related to the patient's environment.
• Coordinate patient care effectively with a multidisciplinary team, including physicians, social workers, and other healthcare professionals.
• Participate actively in the hospice program's quality assessment performance improvement (QAPI) program.
• Conduct follow-up visits as needed to monitor patient progress and adjust care plans.

Required Qualifications

• Active Registered Nurse (RN) license.
• Minimum of one (1) year of recent Hospice experience.
