Shift: Monday - Friday 8:00am - 4:30pm

Position Summary: The Health Systems Navigator provides comprehensive, person-centered case management to participants in View email address on ziprecruiter.com’s supportive housing and Safe Parking programs. This position supports supportive housing residents in maintaining housing, health, and quality of life while supporting Safe Parking participants who are experiencing homelessness, in building stability and working toward permanent housing or other positive destinations.

A primary focus of this position is health and healthcare navigation, particularly for supportive housing residents with complex or ongoing medical needs. The Navigator coordinates alongside healthcare, behavioral health, hospice, and community providers; helps participants navigate healthcare systems and benefits; and addresses health-related barriers that may impact housing stability.  

Preferred Qualifications:

• Degree in Social Work, Counseling, Public Health, Nursing, or another medical program preferred. Can substitute years of experience for degree requirements. 
• Two years of case management or social services experience; 
• Medical field experience and understanding of medical terminology preferred. 
• Must demonstrate knowledge of healthcare treatment and intervention modalities, community resources, treatment planning and the social determinants of health.
• Knowledge of and experience in using trauma-informed care principles required. 
• Demonstrated experience in mediation between persons exhibiting escalated behavior required. 
• Must have valid Oregon Drivers license; must have been a licensed driver for at least 2 years; must have a driving record that permits coverage under View email address on ziprecruiter.com’s company insurance.

Essential Job Duties Include (But Not Limited To):

Data, Documentation, and Compliance

• Maintains thorough and accurate documentation of client interactions, services provided, and progress toward goals.
• Submits all data, case notes, and reports as required by program and grant guidelines. 
• Ensures all Personal Health Information (PHI) is handled in compliance with HIPAA standards.
• Participates in training initiatives, including the Community Health Worker Training Initiative and Learning Management System development, to enhance team capacity.

Relationships & Engagement

Builds and maintains strong, respectful, and professional relationships with participants, healthcare providers, community partners, and View email address on ziprecruiter.com team members. • Builds trusting relationships with participants while maintaining clear and healthy professional boundaries.

• Builds and maintains collaborative relationships with medical, behavioral health, hospice, housing, and other service providers involved in participant care.

• Advocates alongside participants, helping them navigate relationships with systems and providers.
• Facilitates warm handoffs and meaningful connections with other organizations. 
• Approaches conflict, challenging interactions, and difficult conversations with curiosity, calm, professionalism, and a focus on preserving relationships whenever possible.

• Participates in case conferencing and interdisciplinary team meetings to coordinate client care and share insights on best practices for health and housing stability.

Stabilization 

Supports participants in building and maintaining stability by addressing the interconnected health, housing, and practical needs that affect their ability to remain housed, move towards housing, and experience greater independence and the participants/residents identified version of a quality of life. 

• Provides proactive, case management that identifies needs, barriers, and emerging risks and supports participants in developing and achieving their goals. 
• Provides health-focused stabilization for supportive housing residents, including healthcare navigation, benefits navigation, coordination with medical, behavioral health, hospice, and other providers, and support with appointments, transportation, medication coordination, and other health-related needs. 
• Helps residents navigate complex healthcare systems and transitions in care, advocating alongside them to access appropriate services while remaining within the non-clinical scope of the role. 
• Supports supportive housing residents in maintaining housing by addressing health, daily living, resources, and other barriers that may affect housing stability. 
• Provides mobile case management to Safe Parking participants experiencing homelessness, with a focus on stabilization, connection to healthcare and community resources, benefits, and identification, and movement toward permanent housing or other positive destinations. 
• Assists participants in accessing resources that support stability, including healthcare, behavioral health, substance use services, benefits, transportation, food, employment, identification, and other basic needs. 
• Coordinates services and resources across systems to reduce barriers, strengthen support networks, and promote continuity of care. 
• Responds proactively to changes in a participant /resident’s circumstances and collaborate with participants and appropriate providers when additional support or intervention is needed. 
• Supports participants in developing the skills, resources, and connections needed to navigate systems and maintain stability with increasing independence.

Supervision & Guidance

• This position operates under general supervision and will receive regular direction, support, and performance feedback from leadership.
• The role requires initiative, reliability, and regular communication with supervisors regarding project progress, challenges, and development needs.

Physical Requirements 

• This position requires walking long distances outdoors in adverse weather conditions.
• Must be able to go up and down stairs.
• Must be able to push, pull, and/or lift 50 pounds or less.

Work Environment 

• Work is performed in close quarters with vulnerable populations, including individuals without stable housing who may be experiencing mental health and/or substance use challenges.   
• Indoor/outdoor work environment with frequent interruptions, competing demands, and occasional unpredictable behavior.   
• Travel between Church at the Park–managed sites, community partner agencies, and encampments throughout the area
