Mobile Habilitation Specialist — Direct Care (IDD)
MHMR of Tarrant County
MHMR of Tarrant County is seeking a Habilitator to provide direct care in nursing homes for adults and children with intellectual and developmental disabilities. The role includes scheduling, transporting clients, and coordinating with health care providers. You will maintain health documentation, support nursing staff, and travel daily throughout Tarrant County using your own vehicle.
