What You ll Be Doing
Conduct comprehensive health assessments
Diagnose and manage acute and chronic conditions
Perform medication management and care plan updates
Coordinate care with nursing staff and facility teams

Support families through ongoing communication
What We re Looking For
Active NP (FNP or AGNP) or PA license
Experience in geriatrics, long-term care, or primary care preferred
