Job Title: Physical Therapist – Home Health Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Reliable transportation, current driver's license, and agency-required liability insurance Description: Home visits are required for this position. There will be periodic office meetings. The therapist must have reliable transportation and a current driver's license with agency-required liability insurance. Average number of patients per day is 5-6. Each type of visit and activity, including documentation and travel, holds a different point value. The contractor will be assigned between 5-10 patients per day, which equates to roughly 6-8 points. There are no guaranteed hours; if there is a situation where the contractor is unable to meet financial needs due to a low patient census, the contractor should discuss with management the possibility of floating to a nearby clinic to pick up hours. Types of cases include Start of Care and Follow Up visits. First-timers are not accepted; recent Home Health experience is required. Weekend requirements vary by location and specialty and should be discussed during the interview. On-call requirements vary by job/location and should be discussed with management. Must be willing to travel up to 75 miles from home. Mileage will be reimbursed at the IRS standard rate. Travel will be compensated from patient home to patient home, but the commute to and from the first and last patient is not billable. Local candidates are accepted. Pending licenses are not accepted unless the license takes a few days to obtain. All documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation that impacts billing and follow-up patient care may result in cancellation of appointments. The clinician must accept every patient on their schedule regardless of distance from their previous appointment. There are no guaranteed hours with this contract. The clinician must achieve weekly productivity points of 30, which equates to 40 hours of work per week. Floating to other clinics may be required if there are insufficient patients to meet financial needs. Proficient use of the designated electronic medical record system is required within the last six months. Compliance training may be necessary if the required training is not up to date. The contractor must be open to the possibility of on-call rotations. The territory covers Central/South Berkshire County.
