Seven Healthcare are seeking an experienced RN for a Case Management role in Jeffersonville, IN. This travel assignment specifically requires skilled professionals to work in a dynamic environment, focusing on patient-centered care and the coordination of resources.

- Coordinate with physicians, social workers, discharge planners, and claims professionals.
- Engage specialty resources to achieve optimal resolution.
- Keep detailed records of clinical, functional, and fiscal outcomes.
- Identify opportunities for health promotion and illness prevention.
- Act as patient advocate by protecting privacy and confidentiality issues.
- Associates or Bachelor's Degree in Nursing or related field.
- Current, unrestricted state Registered Nurse license (IN or compact).
- Ability to analyze complex medical information and make rational decisions.
- Case Management certification.
