Behavioral Health Case Manager Full-Time or Part-Time

A Great Opportunity Part‑Time or Full‑Time Monday‑Friday 9am‑5pm with some flexibility for evenings or earlier start times. $18–$21 per hour based on education and experience plus a monthly incentive program for exceeding productivity expectations. Our clients are supported by a team of mental health counselors, social workers, certified nurse practitioners, case managers and psychiatrists to ensure the best outcomes for those we serve. Responsibilities Provide therapeutic behavioral services to clients by consulting with a licensed clinician to assist with the individual's needs. Provide service planning for individualized supports or serve as care coordinator of healthcare, behavioral health and non‑healthcare services and develop a treatment plan. Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services. Provide services to individual clients or groups of clients. Promote a positive and cooperative relationship with all outside contacts. Requirements Must be at least 18 years of age. High school diploma plus at least three years of relevant field experience or a Bachelor's or Master's level degree in a related field. Related experience or training with mental or behavioral health conditions. Must have a valid driver's license and reliable transportation. Willingness to travel throughout the assigned service area.
