Therapist - Social Worker

Job Description

Job Summary:
Provide specialized social work services within a hospital setting to support patient recovery and well-being. Collaborate with healthcare teams to facilitate appropriate discharge planning and resource management.

Responsibilities:

• Assess patients' psychosocial needs and develop individualized care plans.
• Administer therapeutic interventions to promote emotional and mental health.
• Coordinate with medical staff to ensure comprehensive patient support.
• Document patient interactions and progress in accordance with hospital policies.
• Maintain licensure and stay updated on clinical best practices.
• Support patients and families through education and crisis intervention.

Qualifications:

• Hold an active GA Licensed Certified Social Worker (LCSW) credential.
• Possess at least 2 years of experience in hospital-based social work.
• Demonstrate clinical proficiency in acute care environments.
• Exhibit strong communication and interpersonal skills.
• Be able to work independently and adapt to fast-paced settings.
• Maintain current knowledge of healthcare regulations and social work standards.
